1) What does loan or lien reported mean? Does it mean a bank has the pink slip?

Not necessarily, but could be. It could be that the dealer got it as a trade-in , wasn't able to sell it, and opted to sell it at an auction. It could be that it was reposessed. If the latter is the case, the previous owner probably was not up on the maintenance if they couldn't afford it. The previous owner could have died and the car was part of his estate. Hard to say. From the report you have, I'd guess a dealer had it and tried to sell it, and was unsuccesful in doing so, and auctioned it to move it off the lot after 3 months.

4) Does Service Facility usually come from the Acura dealership?

In my experience, yes. The mileage is low, so I don't think the car will be problematic. An Acura dealer should be able to tell you exactly what types of service the car has had if it went through a dealership.
