This sounds ridiculous typing this, but I can't figure this out...
I've got a dash cam, its supposed to take 12v direct. I've wired it up to the accessory plugs in the kick panel fuse box with a spade connector:

None of the connections are reading 12v, all of them are around 7v.
EDIT: nevermind... it turns out that bolt at the bottom of the fuse panel isn't a good ground...
