RPS Clutch deffect or installation error? Need Opinion

RPS was installed about 3 months ago.
I had a clip break off the RPS bearing and damage the clutch.
Here are some copy pasted info off of rps's invoice.

this was there notes from when i called in the the issue to send the clutch to them:
NSX- CLIP AROUND THE RELEASE BEARING - WAS BROKEN OFF AND TWISTED - THE CLUTCH WAS WORKING GREAT -JUST HAD A VERY HEAVY PEDAL -WORKING ON THE ENGINE AS SAW IT ....

This was their report after rebuilding the clutch:
FLYWHEEL SHOWED UNEVEN HEAT MARKS - THE RESULT OF THE CLIP COMING OUT HALF WAY - THE DISC SHOWED NORMAL WEAR..BUT SHOULD BE REPLACED - THE PRESSURE PLATE WAS EASILY REBUILD ABLE - THE COLLAR WAS ALSO REPLACED..--- NORMAL COST WOULD BE $400.00 - HAVE NO EXPLANATION AS TO WHY THE CLIP CAME OUT...

NOTE NOTE- HEAVY PEDAL IS THE RESULT OF THE SLEEVE BEING WORN OUT - RECOMMEND THAT IT BE REPLACED BEFORE INSTALLING THE NEW CLUTCH KIT- EVEN IF THE SLEEVE LOOKS OK - THE DEMOTIONS MAY HAVE BEEN
CHANGED - THE SLEEVE COULD HAVE CAUSED EXCESS PRESSURE ON RELEASE BEARING MECHANISM CAUSING IT TO SPIN AND THE CLIP TO COME OUT


Funny thing is the clutch had been heavy as all hell since the day I had it installed at Davis acura by Bernie.
Therefore there comment that the worn out sleeve is the cause of the heavy pressure would have been apparent before replacing the factory clutch that was in there before. That clutch was light as a feather and impossible to have been any heavier than stock.

Acura is saying they cant remember a single time that sleeve ever needed to replaced on the 1000+ clutches they must have done by now.

Even though RPS only charged me half there rebuild cost plus shipping both ways ($350) now there telling me to replace a back ordered $200 sleeve (that acura claims rarely goes bad)
My mechanic says the sleeve has no sign of unusual wear.
Even though I already had the engine out when we discovered the damage I'm gonna have to pay an additional 4 hours labor due to this clutch crap. that has already held me up over 2 weeks waiting for shipping to/from RPS plus inspection/rebuild time (sent it on 7/9 getting it back tomorrow 7/25)

All together this RPS issue is costing me about $900 in addition the top end rebuild / timing belt / hoses / gaskets / which is approaching $4k by itself.

So what could be the fault of the damage?
Anyone else experience this with a RPS clip breaking off that they claim was not a manufactures defect?
Who is at fault here?
Should I replace the sleeve and wait another 2 weeks for the sleeve or just get it back together and hope that it was just a defect RPS is not willing to admit?
