Can this be used on a 95 model? I understand that you can us the later single disc clutch on the 91-96 models if you replace the flywheel which this comes with. So would this be all I would need to convert a 95 to the later clutch style (not including throw-out and pilot bearings)?

Yes it will work on a 95.
