My understanding is the NSX automatic and manual have the same engine, but the automatic version power was detuned for transmission reliability reasons (252 hp vs 270 hp). So is it possible to do a simple stage 1 tune to regain some hp? Only other modification I have is a titanium exhaust.

Not sure what you mean by "Stage 1" but the AT has a 7,500 rpm redline and less cam lift on the VTEC lobes, which are the source of most of the power reduction. I suppose you could do a fuel and timing tune, but there's limited upside with that redline and cam profile. You'll likely find the most improvement (+20ish) by fitting a good set of headers.
To be clear: the AT and MT engine have different physical cams. The difference in HP is at the high RPM range that is available to the MT cams.
The AT engine has a better torque response at lower RPM.
If you don't track your car, the AT engine is likely a better profile for street driving regardless of transmission type.
If you want more HP then a roots supercharger for you. They are getting cheap in comparison to the value of the car. Personally, I had one and it was "meh" for the type of daily driving I did.
